The mission begins with Case and Marshall navigating a Kentucky BioTech facility filled with toxic gas, necessitating gas masks. An elevator malfunction leads to a gas mask breach and subsequent hallucinations. Upon regaining control, locate a locked red-lit door. Use a hatchet found on a mannequin to break it open. Proceed through the hallway, ascend the stairs in the main hall, and access the elevator.

Accessing the Biotechnology Room

Activating the elevator triggers a zombie transformation of the mannequins. Eliminate them with the hatchet. A phone call from the circular desk directs you to the biotechnology room, requiring four director's cards (Red, Green, Blue, Yellow). You'll receive a map to the Yellow card.

Document locations: corner desk, left side near a round table, small center table, and cafe near a sink. Defeat the resulting Mangler Zombie to obtain the Green Card.

Acquiring the Blue Card

Grapple across to the Joint Projects Facility. Answer the phone, locate the camera stands, and eliminate the Mimic to obtain the Blue Card. The Mimic's ability to disappear requires shooting moving objects to trigger its transformation.

Retrieving the Red Card

Follow the red carpets in the East Wing to a room with water and a console. Use the grappling hook to reach the upper area, swim through the red tunnel, and climb the ladders. Eliminate zombies and unlock the door using the blacklight code. Drain the water within 25 seconds by activating three switches (one in the initial room, one in the unlocked room, and one accessed via grappling hook). Defeat the Mangler and its horde to get the Red Card.

Insert all four cards into the security desk. Ride the elevator to the top floor. Answer the red phone and engage the final confrontation with the Disciple and a horde of zombies. The ensuing cinematic reveals the encounter as a hallucination.
